FSMB "Uniform Application." This is a service of the Federation of State Medical Boards and is designed for physicians who wish to apply for licensure in more than one state. Applicants who choose this option may utilize a Federation Credential Verification Service form as well as the Uniform Application. (They are two completely separate processes.)
