A. Application Fee: All fees are non-refundable $80.00

B. Education Requirements: High school diploma or equivalent

C. Experience Requirements: 5,400 hours

D. Examination Information: Application must be approved before the exam can be taken. 

E. Renewal Period:  January 1st through March 1st

F. Renewal Fee: Late $80.00  Renewal $160.00

G. Other Special License Requirements:
Must hold a minimum of $500,000 occurence form of commercial general liability which includes personal injury.
