All licenses offered by this board are subject to primary source verification, meaning we require that transcripts and exam scores be sent directly from the issuing authorities, not forwarded by the applicant.

Renewal
Each license must be renewed annually by the following dates:
  • Athletic Trainer: August 31
  • Audiologist: February 1
  • Audiology Assistant: February 1
  • Limited Speech-Language Pathologist: (not renewable)
  • Occupational Therapist: June 15
  • Occupational Therapy Assistant: June 15
  • Physical Therapist: April 1
  • Physical Therapist Assistant: April 1
  • Speech-Language Pathologist: February 1
  • Speech-Language Pathologist-Audiologist (dual): February 1
  • Speech-Language Pathology Assistant: February 1
The renewal period for each license type opens 60 days before its renewal date.

Late renewals require an additional late fee for each missed year. Suspended and inactive licenses must still be renewed.

Licenses not renewed on time are considered lapsed for 45 days after the renewal date, during which time the licensee may still practice. After 45 days, the license is expired, and the licensee may not practice.

Lapsed and expired licenses may be renewed up to two years past the renewal date. After two years, the license terminates and may not be renewed.

A new application may not be submitted if there is an existing license that is not terminated.
